Personal appearance

• Provide a valid form of government issued Identification. Driver’s license or passport

• Present with a completed document - no blanks

• Sign the document before appearing before the notary, or in the notary’s presence.

• Declare (Acknowledge) signing the document for its intended purpose.

The notary will:

• Witness the document at the time of signing.

• Visually scan the document for completeness (NO BLANKS)

• Ensure that the singer understands what they are signing & signing under their own free will.

• Perform a verbal ceremony if necessary.

• Take an oath or acknowledgement.

• Complete notarial documentation | Certificate

• Record the notarization in a journal.

• Affix signature and seal
